Thank you for choosing Head & Neck Surgery of Kansas City for your ENT needs.   

 

If you are a:

• New patient or you have not been to our office in 3 years, you should be receiving a text message or an email to complete your online health history and registration. 

• Returning patient, you should be receiving a text message or email to update your health history. 

 

If your registration is: 

​

• Complete: Please arrive at the office 5-10 minutes prior to your appointment time with your ID and your current Health Insurance card. 

• Not Complete: Please arrive at the office 30 minutes prior to your appointment time to complete the registration in office with your ID and your current Health Insurance card. 

 

If you have not received a text message or email with a link, please call or text the office at 913.599.4800 to have the link resent.
